Chastity day 37

Hello to all. After 37 days on chastity my balls are visibile bigger. Didn't cum at all. My chastity cage is quite dirty and smells. Should I remove it for cleaning only? I wonder to be on chastity at least a year without cumming. Is it dangerous for my health to be locked for such time without cleaning it? Because I started to like that smell coming from my cage?
